Who is a candidate
We generally consider tirzepatide for adults with a BMI of 30 or higher, or 27 and higher with a weight-related condition such as prediabetes, high blood pressure, sleep apnea or fatty liver.
Tirzepatide is not appropriate if you are pregnant or breastfeeding, have a personal or family history of medullary thyroid carcinoma or MEN2, or have a history of pancreatitis. Your visit includes a full history so we can rule these out before writing anything.
Dosing and the first eight weeks
Treatment starts low and moves slowly — typically 2.5 mg weekly for four weeks, then 5 mg, with further steps only if you are tolerating the medication and results have plateaued.
Most people see the first meaningful change between weeks three and six. Steady, gradual loss of roughly 1 to 2 pounds per week is the goal; faster is not better and usually means you are losing muscle and water.
We check in every four weeks — weight, blood pressure, side effects, and whether protein and hydration are where they need to be.
Side effects and how we manage them
Nausea, constipation, reflux and fatigue are the common ones, and they are usually dose-related and temporary. Smaller meals, more water, more protein and slower dose steps solve most of it.
Call us the same day for severe abdominal pain radiating to the back, persistent vomiting, or signs of dehydration. Those are not 'push through it' symptoms.
